Please bring the following information with you to your appointment:
o Photo ID for you and spouse.
o Social Security cards or statements for EVERYONE on the tax return. This includes dependents.
o All income documents such as W-2, 1099-B, 1099-DIV, 1099-INT, 1099-MISC, 1099-NEC, 1099-R, SSA-1099, RRB-1099, etc. If you think it is important, bring it.
o Routing and bank account numbers if you will be using direct deposit for refunds or direct debit for amounts owed.
o Form 1095-A, the Health Insurance Marketplace Statement, if you purchased insurance through the Marketplace.
o Form SCTC-1 (WV residents) if you are eligible for the Homestead or Senior Citizen Tax Credit. If you have not received your form, contact your county tax office.
o Child/elder care statement showing amounts paid for child/elder care and the providers SSN/Tax ID information if you are eligible for child/elder care credit.
o If you are self-employed, a consultant or a gig worker (Uber, Lyft, Door Dash, etc.) and will be claiming expenses, please bring itemization of expenses.
o Last years entire return
